How to make a Servey.

Hi, I want to know how to make a survey.

Lets say the question is this :

How smart are you?
• Very
• Average
• Stupid

The person would check their answer.

Very would give you 3 points.
Average would give you 2 points.
Stupid would give you 1 point.

There would be about 15 questions like that and they would SUBMIT all their answers, the codes would add up their total of points and then it would tell them something about their number of points such as "You are very smart".
