Survey Software Recommedations

Has anyone integrated any 3rd party Survey software, or solutions onto their site?

I'm looking for some good recommendations with all the bells and whistles.

Is this for a one off survey or a regular type feature?

If a one off survey, I think rather than installing something on your site, you could just use one of the numerous survey sites out there to run it for you.

I used surveymonkey.com when I was at a hosting company. We would use it for post registration survey (how did you hear about us), 30-60-90 day follow surveys and the odd 1-off new feature surveys and the like.

When I was using it I think it was less than $20/month...but, I believe there are free or cheaper versions.

Honestly, I liked it and it was easy to use and to run multiple surveys over long periods of time.
