Order Form & Database

I am trying to make a form that will be on my website that customer can fill out for there order. I also want it link to a database. I want the information to be sent to my email address. How do I do this?

If you need something a little more advanced to place specific orders it will depend on other factors, how does your website currently work, i.e. CMS or not?

Do Phil told you. I think you need a database expert to help or discuss this topic. In my mind, this function should be very hard to do.
