If you need to send a DMCA notice to a company not yet in our database, you can still create a form if you merely add that company to our database first.
The quickest way to find out a company's DMCA contact information is to Google the company's name and "DMCA." If that does not work you'll want to browse to the company's website and look for legal information, or sometimes the information is located in their acceptable use policy or terms of service. Finally, if you cannot find it anywhere, you can simply call or email them and ask, or just send the notice to their adminstrative contact from WHOIS. If a company is uncooperative you can always go over the head and send the request to their provider. An upstream or infrastucture provider for larger web hosts, or a data center provider for smaller hosts who are just resellers.
